IVADINE is a heart medication used primarily to treat certain conditions related to heart failure and angina pectoris, where the primary goal is to lower the heart rate. Its distinctive mechanism and specific indications make it an important option in cardiovascular therapy, especially for patients who cannot tolerate or do not respond adequately to other treatments like beta-blockers.
Mechanism of Action
IVADINE works by selectively inhibiting the "funny" current (I_f) in the sinoatrial node, the natural pacemaker of the heart. This current plays a key role in regulating heart rate. By blocking this current, ivabradine effectively lowers the heart rate without affecting blood pressure or myocardial contractility. This reduction in heart rate decreases the myocardial oxygen demand, particularly beneficial for patients with angina, and improves cardiac efficiency in heart failure patients.
Indications
Chronic Heart Failure (CHF): IVADINE is indicated for use in adults with symptomatic CHF to decrease the risk of hospitalization and to manage symptoms.
Stable Angina: It is used in patients with stable angina to reduce the frequency of angina attacks and to increase exercise tolerance. This is particularly relevant for patients who are intolerant to beta-blockers or for whom beta-blockers are contraindicated.
Dosage and Administration
The dose of IVADINE is adjusted based on the patient's resting heart rate and therapeutic response, with the aim typically to reduce the resting heart rate to a specified target range. It is taken orally, with meals, to enhance absorption.
Side Effects
The most common side effects of IVADINE include:
Bradycardia (unusually slow heart rate)
Phosphenes (a visual phenomenon described as seeing increased brightness in a portion of the visual field)
Hypertension
Dizziness
Less common but more serious side effects may include severe bradycardia, atrial fibrillation, and temporary vision disturbances.
Contraindications
Ivabradine is contraindicated in patients with:
A resting heart rate below 60 beats per minute before starting the medication
Severe liver impairment
Acute decompensated heart failure
Pacemaker dependency (heart rate entirely paced)
